Insurance
To determine your physical therapy benefit and coverage, please call the phone number on the back of your insurance card and follow the prompts for medical coverage and then physical therapy. Should you have further questions regarding your coverage or would like to know if we are a preferred provider in an insurance network not listed above, please contact our office.
